This is an agreement between ______________________, hereinafter referred to as “independent Contractor”, and SamFam, Inc, hereinafter referred to as “Manufacturer”, located at 3614 6th Ave, Altoona, PA 16602.
Independent Contractor agrees to sell products for Manufacturer at the commission rate of 20% of merchandise total for the first 30 items sold, and 25% for all merchandise if more than 30 items are sold in a 30day period.
Independent Contractor agrees to accept custom orders from customer and be responsible for collecting signature release and full payment (including 6% PA sales tax) at the time of taking the order. Payment will be in the form of Cash, Check, MasterCard, or Visa payable to SamFam, Inc.
Independent Contractor will collect orders on a daily or weekly basis. Independent Contractor will mail or deliver order to Manufacturer. Manufacturer agrees to have all orders completed within three working days to be picked up Independent Contractor for delivery or shipped directly to customers, predetermined between Manufacturer and Independent Contractor.
Independent Contractor agrees to purchase demo sales kit (includes 20 full color brochures and demo display including demo plate) for $15.00. Additional demo products can be purchased retail less 25%.
This agreement is not subject to alteration, modification or change, except in writing and signed by both parties.
This agreement shall be effective from the date of acceptance by both parties indicated by signature below.
